• Call 651-224-3024 to complete a phone intake to determine eligibility.
• Setup appointment for tour and presentation.
• Download, print and fill out forms below.

• Send out high school and college transcript requests to previous schools attended.
• Fill out FAFSA on-line. Crown College code is 002383.
• Have a professional complete the recommendation form and send it to the Promise Program (emails are accepted.)
• Gather financial records including: federal tax forms, MFIP grant documents and child support payments if applicable.
• Bring all forms, driver’s license or state ID and social security card to the appointment.
• At the appointment you may bring your completed forms (ask for help at the appointment if you have trouble completing any forms).
• You will have the opportunity to take a tour of our program, view our PowerPoint presentation and interview with our director.
