Undergraduate (Day) Academic Forms
Fall 2008 Registration Instructions: Information about how to register online through Campus Connect. Registration Entry Form: Use this form until the closing day of registration - August 29, 2008. Registration Change Form: Use this form after the closing day of registration. Student Scheduling Worksheet: Use this one-week format to assist you in planning your registration. Data Collection Form: If you are a new student or you are returning after one or more semesters of absence, you must complete and submit this form with your registration entry form. You may also click the link on the left menu bar to submit this form electronically. Major/Minor Change Form: Please note that you must obtain signatures from the Department Chairs. Departmental Application: You may apply to your department after earning 36 semester credits. You must be accepted to your department before registering for credit beyond the ninety-fifth hour. Double majors and some teacher education majors must apply to two departments. Your department may have additional application requirements and procedures. Non-Returning Student Survey: Please complete this form if you are not planning to return next semester. Change of Name & Address: When changing your name, you must present legal documentation along with this form. Transcript Request Form: Request a copy of your transcript by filling out this pdf form, printing and signing it, and then mailing or faxing it to Crown College.
